How do I know if I am eligible for home care?

Simply call our toll-free number at 1.844.702.HOME (4663), and one of our representatives will help you determine if you qualify for home care. We encourage you to contact us before surgery or a hospital stay. You can also tell your doctor or hospital discharge planner that you would like Spectrum Health Visiting Nurse Association to provide your home care services, and he or she can help determine your eligibility.

What happens after I choose services from Spectrum Health Visiting Nurse Association?

Prior to beginning services, your doctor or discharge planner will give you specific care instructions. You will then receive a phone call from one of our representatives at which time we will ask some questions about your home care health needs and schedule your first home visit.

How will my doctor be informed of the care I receive at home?

Your home care clinician will be in direct communication with your doctor, and he or she will coordinate all of your health care needs with that doctor.

How long will I have home care services?

The length of time varies based on your medical condition, and every patient is different. The frequency of your home care visits and the duration of care are dependent on your needs.

How are my service hours scheduled?

Your individual service hours are scheduled by our full-time scheduling team, which also serves as your point of contact for any scheduling changes or cancellations that may occur.

What can I expect from caregivers while they are in my home?

Each caregiver is trained to follow the care plan developed specifically for you by a registered nurse supervisor and your physician. If there is time available after your scheduled activities, home health aides can assist you with any additional activities.
