Home based primary care

Putting patients first is the primary goal of home based primary care, giving you the ability to stay at home for some of your medical care. You’ll start your experience with a face-to-face meeting with your dedicated care manager. Together, you’ll review all program materials and discuss any questions and wishes you may have. This is the person who will help you create a long-term care plan of your choosing so that you can receive coordinated care from a patient-focused, interdisciplinary team.

Features of home based primary care

If you are enrolled in our home based primary care program, you will be assigned one of our physicians as your main provider. In general, you can expect to see your main provider every two to three months but may see other providers of our team more often. Your care team will coordinate your primary care services including care visits, lab work, care management and any social work needs.

Support services include:

    • Access to assistance 24 hours a day, seven days a week
    • Dedicated care managers to help navigate processes
    • Complimentary financial advisors
    • Medication instructions
    • Coordination of medical specialists and additional services
    • Physical, occupational and speech therapy
    • Skilled nursing services
