Core Health
The Spectrum Health Core Health Program is a free, home-based, voluntary program for adults diagnosed with diabetes, heart failure or COPD. Our team will work with you to:
- Learn how to improve your health and quality of life
- Create a plan to help you reach the goals set by you and your doctor
- Help monitor your health and wellness
- Identify community resources that support you
Services are available for English and Spanish-speaking individuals
Program Eligibility
You may be eligible for this free, 12 month program if you are:
- Diagnosed with diabetes, heart failure or COPD
- 18 years of age or older
- Living in Kent County or a patient of Greenville Family Medicine, Greenville IM/Peds, Belding Family Medicine or Lakeview Family Medicine
- Willing to make changes to improve your health
Services are available for English and Spanish-speaking individuals.
You may not be eligible if you are:
- Pregnant
- In long-term care
- Undergoing cancer treatments
- In hospice care
- In end-stage heart failure
- In end-stage kidney disease or receiving dialysis
