Core Health

Home-Based Diabetes and Heart Failure Services

The Spectrum Health Core Health Program is a FREE, 12 month program for adults diagnosed with diabetes or heart failure. A care team will visit you each month to work with you to:

  • Learn ways to improve your health and quality of life
  • Create an individual plan to help you reach the goals set by you and your doctor
  • Help monitor your health and wellness
  • Identify resources in the community that support your well-being

Our goal is to offer you the support and connections that can put you in control of your own health.

Core Health Program Eligibility

You may be eligible for this free, 12 month program if you are:

  • Diagnosed with diabetes or heart failure
  • 18 years of age or older
  • Living in Kent County
  • Willing to make changes to improve your health

Services are available for English and Spanish-speaking individuals.

You may not be eligible if you are:

  • Pregnant
  • In long-term care
  • Undergoing cancer treatments
  • In hospice care
  • In end-stage heart failure
  • In end-stage kidney disease or receiving dialysis
