Core Health

The Spectrum Health Core Health Program is a free, home-based, voluntary program for adults diagnosed with diabetes, heart failure or COPD. Our team will work with you to:

  • Learn how to improve your health and quality of life
  • Create a plan to help you reach the goals set by you and your doctor
  • Help monitor your health and wellness
  • Identify community resources that support you

Services are available for English and Spanish-speaking individuals

Program Eligibility

You may be eligible for this free, 12 month program if you are:

  • Diagnosed with diabetes, heart failure or COPD
  • 18 years of age or older
  • Living in Kent County
  • Willing to make changes to improve your health

Services are available for English and Spanish-speaking individuals.

You may not be eligible if you are:

  • Pregnant
  • In long-term care
  • Undergoing cancer treatments
  • In hospice care
  • In end-stage heart failure
  • In end-stage kidney disease or receiving dialysis
