Core Health Program

Core Health is a free program for adults diagnosed with diabetes, chronic obstructive pulmonary disease (COPD) or heart failure. We have served nearly 3,000 people since the program began over 10 years ago. Our services are personalized to meet your unique needs, helping you make healthy changes in your life—one day at a time, one choice at a time. Our dedicated team will work with you in your own home to:

  • Improve your health
  • Increase your fulfillment
  • Create a plan
  • Find local resources
  • Monitor your progress
  • Reach your goals

Services are available for English and Spanish-speaking individuals.

Program eligibility

You may be eligible for this free, 12 month program if you are:

  • Diagnosed with diabetes, heart failure or COPD
  • 18 years of age or older
  • Willing to make changes to improve your health
  • Living in Kent County or a patient at Spectrum Health Greenville Family Medicine, Spectrum Health Greenville Internal Medicine & Pediatrics, Spectrum Health Belding Family Medicine, or Spectrum Health Lakeview Family Medicine

You may not be eligible if you are:

  • Pregnant
  • In long-term care
  • Undergoing cancer treatments
  • In hospice care
  • In end-stage heart failure
  • In end-stage kidney disease or receiving dialysis
