Continuing Care Patient and Family Partner Program
The Continuing Care Patient and Family Partner Program was established in May 2013.
Focus
The PFPP brings the patient perspective to the forefront of:
- Hospice care
- Long-term care
- Rehabilitation and nursing center care
- Visiting nurse association care
Accomplishments
The program has:
- Provided the patient perspective on future building needs
- Helped establish a new admitting protocol for weekend admissions to the rehabilitation and nursing care center
