Referrals and consultations

We emphasize education, treatment and prevention of complications for disorders of the glands, hormones and metabolism. Easy, coordinated access is provided to specialties and services, including heart care, foot and ankle medicine, neurology, eye care, weight management and pain management. We work with patients to explain the importance of exercise, nutrition and blood sugar control for regulating diabetes and improving quality of life. We test the blood glucose levels of everyone admitted to our hospitals. This tells us if patients have diabetes mellitus or if they’re at increased risk for developing it (pre-diabetes). Well-controlled blood glucose levels have been shown to lead to fewer complications, improved healing and shorter hospital stays.

Education

Our education program is accredited through the American Association of Diabetes Educators (AADE). Patients managing diabetes don’t have to do it alone. We provide the necessary tools so patients can successfully self-manage the day-to-day rigors of living with diabetes. A certified diabetes educator will work with your patient to create a plan that provides ongoing support and tools for improved health.

Mental health and counseling

Diabetes can bring unique stressors to patient’s lives, affecting mental and emotional well-being. Counseling services support patients struggling to cope with common mental health conditions and chronic illnesses. These services complement the care patients receive from our multispecialty team. Counseling is covered by most insurance companies. These services are available to patients seen in our clinic. No referral is needed.

Diabetes and pregnancy program

The medical management of diabetes during pregnancy increases the biological, psychosocial and financial demands on a woman and her support system. Our diabetes and pregnancy program assists with the management of all types of diabetes and dovetails with routine obstetrics and gynecology pregnancy care. Our comprehensive team offers a patient-centered approach that enhances a woman’s ability to manage her diabetes and the lifestyle changes needed to ensure her and her child’s well-being.
