Over the past seven years, Spectrum Health has successfully led a remote patient- monitoring program focused on chronic-disease patients with the primary diagnosis of heart failure, COPD and diabetes. By leveraging technology, daily monitoring of key health metrics and behavioral coaching this program effectively reduces costly hospital readmission's and can significantly increase a patient’s quality of life.

Telehealth

Remote patient monitoring is the process of using remote monitoring devices to help monitor your residents and significantly reduce re-hospitalization and emergency room visits. Monitoring devices can track blood pressure, heart rate, weight, blood glucose, and oxygen saturation levels.

MedNow uses technology to bridge the distance between your residents and a health care provider. The consultation is similar to a typical office visit, but uses a secure video connection. It is available wherever your residents have Wi-Fi access. All interactions are secure and HIPAA compliant to ensure confidentiality.

Video visits can take place via webcam anywhere a strong Internet connection exists. Such visits provide direct, real-time access to a primary care provider for consultation in regards to such non-life-threatening conditions as:
