Spectrum Health Reed City Family Practices are taking a new approach to patient care. Designated by Blue Cross Blue Shield of Michigan as patient-centered medical homes, we are transforming our offices to to serve as your consistent, reliable source for preventive care, as well as for treatment of illness and chronic medical conditions. The focus of a medical home is you and your relationship with your doctor. We want to be the first place you think of for health care.

Your medical home is a place you feel comfortable and confident - both in the relationship with your doctor and the quality of care you receive. You and your doctor together will focus on your specific health care needs and ongoing wellness.
With the patient-centered medical home model, care teams are created that work with patients to improve health, keep them healthy and monitor their care on an ongoing basis. Each family practice site coordinates your health care using registries to track conditions and ensure that patients are receiving the care they need. They are able to offer patients more access to the care team; coordinate complimentary care (such as nutrition counseling) and help patients better manage conditions like asthma and diabetes.
The patient-centered medical home is changing the shape of health care delivery. Benefits include:
- Collaboration. You work with your physicians to determine a plan of care.
- Coordinated Care. If you need specialty, inpatient or outpatient care, your medical home connects you. This helps prevent delays in treatment, hospital readmissions and miscommunication.
