Not Ready for Home – Inpatient Options

Many times, patients are ready to leave the hospital but are not medically stable enough to return home. Continuing Care offers a variety of service lines to meet the needs of each individual.

The following services are provided in a facility or hospital-like location:

Rehab and Nursing Centers offers patients subacute rehabilitation and long term care. Subacute rehabilitation provides therapy while you are recovering from injury, illness or surgery. Long term care doesn't simply mean nursing home, it is care to help residents restore and maintain the highest possible level of emotional and physical well-being.

Center for Acute Rehabilitation is a hospital-based service for patients who will benefit from more intensive medical rehabilitation prior to returning home.

Special Care Hospital is an acute care hospital that specializes in caring for patients who are not medically ready for transfer to a rehabilitation hospital, skilled nursing facility, long-term ventilator facility, assisted living or home.

Hospice provides end-of-life care for the patient in any setting. Our Hospice care team partners with your family to help you through one of life's most difficult times.

Palliative Care is a supplemental treatment that offers relief from troubling conditions, helping patients live more comfortably and enjoy improved quality of life.
