Core Health Program

Core Health Program

Core Health is a free, 12-month program for adults with diabetes or heart failure. We provide one-on-one visits to your home from a registered nurse and community health worker. Working with your doctor, we will help you:

  • Understand your health condition and how to access care
  • Manage your condition and learn how to work with the health care system
  • Improve your health through lifestyle changes
  • Set personal health goals

We accomplish these tasks by:

  • Educating you based on your interests
  • Mentoring you to help you meet your goals
  • Connecting you with community resources
